HEALTHY LIVING: Free Training with the California Healthier Living Program

Wanted: Volunteer Program Leaders
for Faith-Based Partnership

Do you want to promote good health in our community?
Kaiser Permanente, in collaboration with Los Angeles Oasis – Wise & Healthy Aging is looking for Volunteer Program Leaders to facilitate the California Healthier Living/Living Well With Chronic Conditions program at religious congregations in our community.

If you are interested in promoting health initiatives, or if you have
participated in the program in the past, you may be eligible
to become a Program Leader.
Program Leaders receive four days of local training (cost covered by Kaiser), along with all the program materials and ongoing support.
You will need to commit to facilitating at least one workshop a year.

Training Dates: June 12, 13, 19, and 20, 2018
*Please note: attendance is required on
all four full days (no exceptions)

Time: 9:00 a.m. to 4:30 p.m.
